Fees Without Insurance:

$175 for Intake (this first session gathers history and gets to know you and your goals)

$125-$145 per 50 minute session

$165 per 60 minute EMDR session

Insurance:

Angela currently accepts the following insurance:

Aetna, Blue Cross Blue Shield, Cigna and Evernorth, Optum, and UnitedHealthcare UHC | UBH.

If we are out-of-network with your insurance plan, we are able to provide you a Superbill that you can submit to your insurance provider for reimbursement. The decision for reimbursement is made by your insurance provider.

We recommend asking your insurance provider some questions beforehand to help determine your benefits:

  • Does my health insurance plan include mental health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

  • How much does my insurance reimburse for an out-of-network provider for CPT codes 90834 and 90837?

  • How do I submit a Superbill?

    Remember-you are responsible for knowing your insurance plan and coverage.

Keep in mind: Reimbursement requires a diagnosis - a Superbill, just like any other paperwork submitted to insurance for reimbursement, requires a clinical diagnosis as classified in the Diagnostic and Statistical Manual of Mental Disorders (DSM–5) and the International Classification of Diseases (ICD-10).

Payment

Obear Counseling accepts payment from: Health Savings Account (HSA), Flexible Spending Account (FSA), cash, and all major credit cards. Payments will be processed and accepted directly on the patient portal or in-person. Loveland Foundation vouchers are also accepted by Alexis Fitzpatrick.

Good Faith Estimate 

I understand that I will be responsible for all charges related to the services provided to me by Obear Counseling. 

I understand that the charges presented to me are due in full on the day of service. I also understand that these charges are solely in relation to professional services provided by the clinician. 

I will also be responsible for all missed appointment fees and late cancellation fees billed to me separately from Obear Counseling. 

The client or parent/guardian of the client (age 16 and younger) certifies that he/she has read and hereby accepts the terms and conditions listed below. As per the client’s treatment plan, the following information will be provided: the client’s diagnosis, the service code for the session, and the prescribed frequency of treatment. Treatment will be provided either in-person in Knoxville or Murfreesboro, TN or via Telehealth at the client’s discretion. 

Clients may contact their therapist with any questions regarding costs.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged for the full rate of the session.
